The Military Personnel Flight is the strategic manager of military personnel programs for all Air Force units and some tenants, whether on the base or geographically separated from Buckley SFB.

In an effort to better serve our customers and value your time, we have implemented appointments as a primary means to issue ID cards and DEERS record updates. Walk-Ins will still be welcomed and accommodated based on availability however, those who walk-in may experience longer wait times. Pin Resets and CAC Issuance/Work Stoppage will continue to be expedited to the next available machine.

Official Passport Services

(We Do Not Process Blue Tourist Passports)

Required Documents

You will need the following documents to process your official passport application at an appointment.
Once you have all these documents ready, please call or email me to set up a passport appointment.

The following list of items are required to process a passport:

  • 2 passport photos
    You can get these from Walgreens at your cost or you can go to the Public Affairs office on base (Bldg 730) for free with an appointment – 720.847.9226.
  • Current/Expired Official/Tourist Passport if you’ve previously had one. If not, the DS-11 is required for whom do not have a Tourist Passport.
  • Marriage Cert (if applicable) if renewing your old passport the DS-82 is used.
  • Birth Certificates
  • DoD CAC ID Card and Drivers License
  • Passport Application

    go to https://travel.state.gov and complete the application for a passport.

Official Orders Are Required

With the dates you are going and the country you are going to that requires an official passport IAW the Foreign Clearance Guide.

If you do not have your orders and still wish to try to proceed to process the application, then you will need an approved Memorandum in Lieu of Orders. This must have supporting documentation with it too. This completed memorandum and supporting documentation must be sent to me first so we can submit it to HHQ (DET) for them to approve/disapprove.

Additional Information

Renewal of ID/CAC Cards

Renewal of ID/CAC cards are now being done within 60 days prior to the expiration date.
(Exception: Deploying Members with orders whose expiration date is within the dates of the deployment)

Lost/Stolen OR Confiscated ID Card

If you lost your ID card, you will need to bring a “Lost ID Report” to our office and two forms of ID. For lost/stolen CAC cards, you will need one of the following items below in addition to 2 forms of identification, one of which must be an unexpired photo ID. To obtain a Lost ID report, please go to the Buckley AFB Visitors Center which is currently at the Mississippi gate

Contractors for ID/CAC card

Ensure your Trusted Agent (TA) has you in the TAS system. Contractors must have TWO (unexpired) forms of ID. One must be a state/government issued photo ID. If going overseas, please ensure you have your current (within 30 days) LOA.
**If this is your First CAC/ID card, please ensure you have arrangements to access the installation. Any documents must be Original/Certified. Photocopies are NOT acceptable.**

Military Retirees

Initial Requirement: Retirement Orders *Reserve/National Guard Retirees: Orders/Letter transferring to Retired List and authorizing retired pay. If Medicare eligible, the Medicare Card along with the two (unexpired) forms of ID.

Turning 65 – Tricare For Life. If you are turning 65 years old and will be on TRICARE for Life, you must provide your Medicare card showing Part A along with TWO (unexpired) forms of ID.

Adoption of Children Pre-Adopted State: Sponsor will need a Placement Letter for Adoption from the court in process, Birth Certificate, and Social Security Card. **Documents must be Original/Certified. Photocopies are NOT acceptable.**

Final Adoption: Court Order, Birth Certificate, and Social Security Card. **Documents must be Original/Certified. Photocopies are NOT acceptable.**

Incapacitated Child: Initial ID cards will need to be completed at the Sponsor’s ID card office. For renewal of any sort, you will need to have a DFAS Re-determination completed in order to be issued a new Dependent ID card. You will need to complete the DD Form 137-5 and submit it to the DFAS. Once completed, we will need the DFAS letter of approval, Dependent Military ID Card, and another valid form of identification as defined by the Department of Homeland Security.

Dependent Ward: An initial Dependent Ward application will need to be completed. Application consists of completion of the DD Form 137-7 and submission of form and all required documentation to DFAS. DFAS will mail a Letter of Approval to the sponsor.

Once received, sponsor will need to provide DFAS Letter of Approval, Court Document placing the child in member’s household, Birth Certificate, and Social Security Card. (Applications can be picked up at the ID Card Section office). **Documents must be Original/Certified. Photocopies are NOT acceptable.**

Civilian Retirees

Civilians retiring must have SF Form 50 and have TWO (unexpired) forms of ID (No Badges). One must be a state/government issued photo ID.

Civilian Employees (Overseas Position) Provide DD Form 2365, SF Form 50 annotating that person is assigned to an Overseas Emergency Essential Position, and two (unexpired) forms of ID (no badges). One must be a state/government issued photo ID.

For renewal of any sort, you will need to re-accomplish the DFAS application process, obtain another Letter of Approval in order to be issued a new Dependent ID card. Dependents

Adding spouse: Must bring marriage certificate and two forms of ID. Sponsor must be present.

Adding child: Sponsor should bring a birth certificate and a social security card. If birth certificate is unavailable then we can use a proof of birth from the hospital. If SSN is unavailable we can still add the child to DEERS however you will need to come back in the future to add the SSN.

Name Change for Children: Provide official Original/Certified legal documentation (Court Order or Birth Certificate) for name change of children.

Designated Agent Letter

The Designated Agent letter is issued to an authorized person designated by the military sponsor (or surviving spouse). This letter allows that person access to the Base Exchange, Commissary, and Pharmacy for shopping on behalf of the sponsor.

This letter is only valid for one year at a time. Only one person may be designated per household. Authorization cannot be granted solely for customer convenience – an extreme hardship must exist and be validated. Dual military couples (active duty) with children in the household and single parents (active duty) with children in the household may also have a designated agent when a family care plan is in effect.

Base Exchange and Commissary personnel have the right to limit purchases made in their store. Any suspicion that someone is attempting to make purchases in a fraudulent manner (for other than the sponsor and their household) may result in confiscation of the Designated Agent letter and suspension of privileges in accordance with 460th Force Support Squadron Standard Operating Procedure.

The following current supporting documentation must be presented each time a letter is issued and/or renewed: Authorized patron’s presence or valid Power of Attorney: If the authorized patron is unable to be present at the time of issuance, the person obtaining the agent card must have a current Power of Attorney authorizing them to conduct actions pertaining to the military on behalf of the sponsor or surviving dependent.

Authorized patron’s ID card: The authorized patron is the military or retired military sponsor or the surviving dependent of the sponsor. The ID card must not be expired.

Letter from attending physician: When the sponsor is physically unable to shop for themselves, they must obtain a letter from their doctor stating their incapacitating condition or inability to care for themselves. This letter must be current within the year of obtaining the agent letter. This requirement does not apply to active duty military couples with children or active duty single parents with children.

Copy of current Family Care Plan (Active Duty only): Dual military couples or single parents with children must provide this in order to obtain a Designated Agent letter (usually in cases of deployment, unaccompanied PCS or TDY). The designated agent must be someone designated as a caregiver on the family care plan.

Civilian Retirees ID Cards

What are the operating hours for the Buckley AFB ID Card Section?

Answer: We have multiple ID sections on base however, the 460th Force Support Squadron ID Card Section does accept walk-ins and is located in Bldg 606 and open the following days and hours;

Mondays / Tuesdays / Thursdays / Fridays: 8:00 AM – 3:00 PM
Wednesdays: 8:00 AM – 12:00 PM

My DoD ID card has expired or is expiring soon; what do I do?

Answer: Through June 30, 2021, DoD will temporarily allow USID cardholders to use an expired DoD ID card to access bases and benefits.

Uniformed Services ID Cards USID cards that expire on/after January 1, 2020 may continue to be used to access military bases and benefits. In order to use the expired USID card, the cardholder must have a current affiliation with DoD and must be eligible for benefits. For example:

  • An ID card issued to a spouse typically expires every four years; however, as long as the spouse remains married to the Service member, the spouse continues to have a current affiliation and is eligible for benefits. As a result, a spouse MAY use the expired DoD ID card to access benefits.
  • An ID card issued to a child of a Service member expires when the child turns 21 years old. Unless attending school as a full-time student, approved as an incapacitated dependent, or enrolled in TRICARE Young Adult, the child’s affiliation ends when the ID card expires and is not eligible for continued benefits.
  • As a result, a child who has turned 21 and is not eligible for continued benefits MAY NOT use the expired DoD ID card to access benefits. Please schedule an appt or walk-in into the Buckley AFB ID card facility during the hours above

NOTE: A child under age 21, or who is eligible for continued benefits, MAY use the expired DoD ID card to access benefits.

USID cards are issued to retirees, family members, and other eligible individuals. If you have questions about whether you have, or are eligible for, a USID card, visit the following website: https://www.cac.mil/Next-Generation-Uniformed-Services-ID-Card/.

If you have questions about the status of your benefits, please use DoD’s online self-service resources before contacting DoD and Uniformed Service help desks by telephone. You can login to mil Connect, https://milconnect.dmdc.osd.mil/milconnect, using the DoD Common Access Card or DS Logon account. TRICARE coverage is found within the “Medical, Dental and Pharmacy” drop-down under “the Benefits” tab.

Does my sponsor have to be with me to get an ID Card?

Answer: Yes, with the following exceptions:

  • If the sponsor signs DD Form 1172-2 (Application for Uniformed Services Identification Card/DEERS Enrollment) in front of a Verifying Official, the family member may bring this form unaccompanied by the sponsor.
  • If the sponsor signs DD Form 1172-2 (Application for Uniformed Services Identification Card/DEERS Enrollment) and has it notarized, the family member may bring this form unaccompanied by the sponsor.
  • The family member may sign the DD Form 1172-2 (Application for Uniformed Services Identification Card/DEERS Enrollment) using a general power of attorney (P.O.A.).
  • If the Sponsor visits www.dmdc.osd.mil/self_service and complete DD Form 1172-2 electronically.

Once the DD Form 1172-2 is signed, how long is it good for?

Answers: 60 days from date of sponsor’s signature.

What is the minimum age a dependent can receive an ID Card?

Answer: Based on COVID guidance Age 14, with the following exceptions:

  • Dependent dual active service members
  • Sponsor is a single parent
  • Dependent is not residing in sponsor’s household

How long may a dependent keep an ID Card?

Answer: Up to age 21. At age 21 if the dependent is enrolled FULL TIME in an accredited institution of higher learning, that dependent may have an ID card until the date of graduation of their 23rd birthday, whichever occurs first.
Exception: if the dependent is incapacitated, a child dependency packet needs to be completed. This packet can be obtained from our office and then forwarded DFAS.

What is needed to establish a dependent as a full-time student in DEERS?

Answer: A Letter from the school registrar’s office stating that the dependent’s (student) full-time status and an anticipated graduation date.

How is a spouse added to DEERS?

Answer: The sponsor will bring in the marriage certificate, birth certificate, social security card and picture ID (i.e. Driver’s License, State ID, etc.). For non U.S. Citizens a temporary ID number (TIN) will be used in lieu of a social security number. When foreign documents are presented, an English translation is required, no exception. (check with army OneSource or U.S. Department of State for assistance).

How are children added to DEERS?

Answer: It is dependent on the following:

  • A female sponsor needs only bring the children’s birth certificate and Social Security card.
  • An unmarried male sponsor must have the child’s birth certificate, Social Security card, and judicial determination (i.e. court-ordered child support or divorce decree naming children as his own). If no judicial determination has been made, forms are available at an ID Card Center that must be sent to DFAS for approval.
  • If a male sponsor is married and his wife is already on DEERS, he only brings the child’s birth certificate and Social Security Card.
